You can request copies of your medical records by completing an Authorization for Release of Information form and returning to Carrollton Regional Medical Center.

To request copies of your medical records, please download, complete, and sign the Authorization for Release of Information form for your Carrollton Regional Medical Center facility. Completed forms may be mailed to 4343 N Josey Ln. Carrollton TX. 75010, attn Medical Records Department.

You can also fax the completed forms to the facility’s Health Information Management Department at 972-394-2351.
