For Patients
Carrollton Regional Medical Center now offers an online records request tool that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

If you are unable to complete your request online, complete the authorization form to give us permission to release your medical records to you or your provider. When you have completed the appropriate form, please email, mail, or fax the form, along with a copy of photo ID to:

For Third Party Professionals
Chartswap is an all-in-one digital medical record retrieval platform that enables third-party requesters (i.e., law firms, insurance companies, and third-party retrieval companies) to request, track, pay for, and securely download records in one centralized location.
Please allow 5 to 14 business days for completion of a medical records request. Please be aware that it can take up to 30 days as allowed by law. Continuing care releases are given priority. If you need medical records for an upcoming appointment, please let us know, and we will expedite your request.
Radiology Images
For radiology images performed at the hospital, please call the Radiology Department at 972-294-2497. The completed Authorization for Release of Information, above, and copy of picture ID is required. Radiology fax number is 972-492-3185.
Billing Records
For billing records, please contact 972-512-1400. You can also complete the authorization form above and submit to billing@crmc.health along with a copy of your photo ID..
